Job Description

JOB SUMMARY

Under the Sioux Lookout Area Primary Care Team (SLAPCT) and within the scope of practice of a Registered Nurse, the RN will be responsible for providing primary health care to clients within the Sioux Lookout area. The RN works with the client and the community to promote healthy lifestyles and disease prevention, as well as collaborates and consults with members of an interdisciplinary team to address issues experienced by clients and within the community. The RN will provide holistic and integrated care to all ages within the Sioux Lookout catchment area, including the 33 First Nations communities.

The RN is directly accountable to the PCT POD Manager.

There are three positions available and are for two year terms.

Minimum Qualifications:

  • Persons of First Nations ancestry will be given preference (OHRC, Part II, Special Employment).
  • Registration in good standing with the College of Nurses of Ontario
  • New graduates are welcome to apply, however preference will be given to those with experience working as an RN in a clinical setting.
  • Knowledge of wound and foot care is an asset; training will be provided.
  • Awareness of and sensitivity to the health issues of a diverse community and experience working with First Nations communities is an asset.
  • Demonstrates ability to work effectively in a multi-disciplinary
  • Ability to provide coaching, mentorship, and guidance to community-based health
  • Excellent relationship-building skills to initiate and maintain working relationships with management, and internal and external
  • Excellent verbal and written communication
  • Experience with electronic medical records, health information databases and strong computer skills.
  • The ability to function effectively during periods of rapid change and
  • Knowledge of or willingness to learn about communicable diseases specific to the population we serve.
  • Ability to communicate in one of the First Nations dialects of the Sioux Lookout region is an
  • Must be willing to travel by various modes
  • Must have a valid Driver’s License
  • Must be willing to relocate and live in Sioux Lookout (or within a daily commuting distance)
  • The ability to perform the requirements of the position on a regular basis.

Roles and Responsibilities:

  • Knowledge to support the Management of Chronic Illness in Clients, specifically Wound Care, Hep C and Diabetes.
  • Assess, treat and monitor clients specific to their healthcare needs.
  • Educate and counsel clients and their caregivers to promote function and independence including health promotion and injury prevention.
  • Work with clients and communities to promote healthy lifestyles, disease prevention and community
  • Performs administrative duties in support of the organization’s goals and
  • Perform other duties as assigned within the scope of the position
