POSITION DESCRIPTION:
The primary role of the nurse is to provide patient-centered, collaborative mental health assessment and intervention and referral to the most appropriate hospital or community services and supports. The nurse will receive referrals from multiple sources, liaise with hospital and community resources and supports as needed in establishing patient care plans. All services are provided in accordance with the Standards of Practice established by the College of Nurses of Ontario and with current legislation.

DUTIES AND RESPONSILBITES:
* Receive patients through the central connect work queue
* Review each patient's historical clinical information to understand presenting issues and any themes in histories that are contributing to their current concerns
*Complete screeners to assist in program matching clients
*Support referrals which require more medical expertise, including but not limited to eating disorders, ACTT, EPI or those with complex comorbidities or complex medication profiles
*Referring clients to the most appropriate Mental Health and Addictions services within or outside of Lakeridge Health
* Be familiar with community agencies/services and engage in positive working relationships with same
* Conduct comprehensive assessments of adults and youth experiencing a range of emotional, psychological, addiction and/or psychiatric issues
* Develop a recovery-focused action plan to meet the person's immediate needs and provide them with resources while they are awaiting services
* Document the assessment, plan, intervention, consent as applicable, disposition, data and other information.
* Work collaboratively with other internal and external services
* Participate in hospital/program quality improvement activities
* Collect and compile program statistical information
* Other duties as required

QUALIFICATIONS:
* Registration with the College of Nurses of Ontario (in good standing);
* A current B.C.L.S. certificate is required
* BScN is an asset
* Minimum 3 year Mental Health/psychiatric experience with individuals with mental illness and/or addictions is preferred
* CNA certification in Psychiatric/Mental Health Nursing is considered an asset
* Demonstrated commitment to maintaining Reflective Practice Standards as outlined by the
CNO;
* Demonstrated excellent knowledge of, and experience in group theories and dynamics;
* Demonstrated knowledge of Community Mental Health Resources;
* Completion of a Non-Violent Crisis Prevention or equivalent course, or within 1 month of hire.
* Completion of a Mental Health Certificate Program preferred with expected completion within 3 years of hire.
* Demonstrated knowledge of the Mental Health Act, Trauma-informed care and Psychotropic medication.
* Demonstrated understanding of approaches to assessing and management of suicide risk
* Demonstrated ability to intervene effectively in clinical situations requiring the management of disturbed behaviour
* Ability to conduct Mental Health Assessment with excellent interviewing and communication skills;
* Excellent communication and interpersonal skills to develop a therapeutic Nurse-Client relationship and work within a collaborative and diverse interprofessional team;
* Effective leadership, decision-making, organizational and problem solving skills are required as demonstrated by previous experience in a charge/team lead role.
* * Ability to work with clients of diverse cultural and social backgrounds is necessary
* Bilingualism or proficiency in a second language would be an asset;
* Must have satisfactory performance record;

* Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-services in this area; and;
* Proven good attendance record with capability of maintaining this same standard;

CONDITIONS OF EMPLOYMENT:

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and the Ontario Nurses Association (RN Bargaining Unit Local 051).

Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
