UHN is a caring, creative place where amazing people are amazing the world.Union: ONASite: Toronto General HospitalDepartment: Diabetes/Endocrine ProgramReports to: Nurse ManagerSalary Range: $50.68 - $68.34 hour (Start rate determined in accordance with collective agreement requirements)Hours: 37.5 Hours/Week; Shifts: Monday - Friday, Days.Status:Temporary Full-Time (approx. 1 year)Closing Date: July 4, 2024As a member of the Diabetes/Endocrine Program multi-disciplinary team, the PATIENT EDUCATOR practices in accordance with relevant legislation/standards, meeting the legal and ethical requirements of the profession. The principal responsibilities include: providing patient education in diabetes for both clinic outpatients and in-patients preparing for discharge home. Duties include teaching medication self administration by injection, insulin pump; patient self-measurement of glucose levels using continuous and intermittent methods of monitoring.Adjustment of Insulin and oral hypoglycemic agents using a Medical Directive, applying knowledge of nursing science, theory, skill and critical decision making in the provision of compassionate, collaborative and competent care.The skills are practiced in accordance with the UHN Professional Practice Model for Nursing and the mission and vision of nursing at the UHN; ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Nurses of Ontario and the UHN policy and procedure; demonstrating commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession; performing reflective practice, analyzing and evaluating nursing knowledge and modifying practice accordingly to improve the quality of nursing and patient care; performing cross-functional and other responsibilities, as assigned and/or requested.Current Registration with the College of Nurses of Ontario requiredBasic Cardiac Life Support Certification requiredFlexible and able to work effectively and efficiently within a interdisciplinary patient care teamMust be able to meet the physical requirements of the positionExcellent interpersonal, communication and decision making skillsExcellent organizational and time management skillsCDE requiredExperience working in a fast paced ambulatory setting that includes patients with complex medical needsHealth assessment skillsExperience with patients with diabetes, lipid disorders, thyroid disease, osteoporosis and disorders of other endocrine glands preferredMust be flexible and able to adapt to a fast-paced changing schedule of an ambulatory clinicIn addition to working alongside some of the most talented and inspiring healthcare professionals in the world, UHN offers a wide range of benefits, programs and perks.
