Job Description

Reporting to the Director of Clinical Services the NPwithin the Diabetes Program is a crucial role in the comprehensive care ofpatients with diabetes, focusing on both treatment and education to empower patient’sself-care to manage their condition effectively. The NP will workcollaboratively with the interdisciplinary team of professionals includingPhysicians, Educators, Dietician, Pharmacist and Social Worker in providing aseamless outpatient journey for all patients presenting to the clinic. The NPwill practice to the full NP scope of practice by ordering and interpretingdiagnostic tests, prescribing medications, education, monitoring, referringpatients when required to other specialists and all other duties as assignedwithin the CNO Standards of Practice, Nurse Practitioners and policies of theTADH.

Position Responsibilities:

  • Provide comprehensive client care usingadvanced assessment skills and sound knowledge of clinical therapeutics
  • Direct, provide and evaluate as holistic clientcare based on advanced nursing and medical management knowledge and skills.
  • Perform and document comprehensive clientassessments based on thorough history taking and physical examination.
  • Identify and analyze client’s physical andpsychological health care problems.
  • Develop plan of care in collaboration with theteam of professionals within the Diabetes Program
  • Identify health education needs of clients andfamilies independently or in collaboration with the interdisciplinary team.
  • Identify, order and prioritize appropriatediagnostic test, medications and procedures.
  • Interpret laboratory/radiology and other testresults.
  • Communicate with and interpret toclients/families and community health care members diagnosis, treatment plan,and related implications.
  • Accurately document assessment, plan,intervention, and evaluation of client/family responses in hospital chart.
  • Liaise with staff regarding clients tocommunicate information and facilitate follow up care and treatment.
  • Direct referrals to the appropriate health careprofessionals or services.
  • Provide individual counselling/instruction tofacilitate client/family understanding of Diabetes.
  • Assist individuals and families in coping withDiabetes management and treatment plan by guiding and supporting whileproviding an environment that promotes independence.
  • Capture relevant and required data as definedby the Ontario Health, HQO, Accreditation or other quality improvementstrategies.
  • Align improvement projects with theorganization and monitor or adjust to achieve greater outcomes.
  • Educate and promote health, safety and wellnessin the work place.
  • Perform other duties as required.

Position Requirements:

  • Completion ofaccredited post BScN, NursePractitioner program
  • Registered and ingood standing with the College of Nurses of Ontario
  • Minimum of 2years’ experience working with clients with acute and complex medicalissues.
  • Past experienceworking as an NP in an outpatientsetting considered an asset
  • Experience inprogram development and education
  • Demonstratedknowledge of evidence based practices
  • Effective timemanagement and ability to prioritize workload
  • Must possess theability to work with minimal supervision and must follow standardizedprocedures.
  • Should beadaptable to cope with changing contacts with people, situations andconditions.
  • Must haveemployment history which demonstrates regular attendance at work.

  • Starting salary of$60.99/hour; $118,930.50/year (As per Step 1 of the ONA collective agreement.)
