Reporting to the Manager of Home and Community Care, the Licensed Practical Nurse (LPN) is responsible for delivering nursing to Kwanlin Dün First Nation citizens, families, and community with complex care needs. This position provides a strong focus on clinical assessments, chronic condition support, coordination of care services, advanced foot care, and health promotion. The LPN provides low barrier nursing care services within the legislated scope of practice, standards of practice, organizational policies, procedures and competencies.
The Home & Community Care LPN is an active member of a collaborative multi-disciplinary team who provides person-centered care services to Elders and community members. Working in a diverse and busy environment, the LPN engages clients and supports them in accessing the programs and services delivered by Kwanlin Dün First Nation, as well as clinical and other community-based agencies needed in effort to achieve client identified health goals.

Required knowledge and skills (what we asses on/may interview on):
Knowledge of Kwanlin Dün First Nation governance structure, our community, heritage and culture, social structure, language and traditions, goals, and aspirations are essential for success in this position.
Conditions of employment: Possess a valid Class 5 Driver’s License with a clean Driver’s Abstract (for KDFN Gov vehicle insurance purposes) Access to reliable transportation; current CPR “Level C” and First Aid certificates (or obtain within two months of the date of hire); Tuberculosis (TB) clearance; acceptable criminal record and a Vulnerable Sector Security Clearance prior to commencement of work; Transportation of Dangerous Goods Certificate (or obtain within two months of the date of hire).
