JOB TITLE:                        INTAKE WORKER/HEALTH PROMOTER STATUS:                            FULL TIME DEPARTMENT:                  COMMUNITY HEALTH PROSPERITY PROGRAMLOCATION:                        4421 QUEEN STREET, NIAGARA FALLS, ON HOURS OF WORK:            40 HOURS PER WEEK POSTING DATE:                  CLOSING DATE:                   WAGE RANGE:                  $68,000 - $70,000 + IN LIEU OF BENEFITS POSITION OBJECTIVE:The role of the health promoter for the Community Health Prosperity Project (CHP) is to work with patients who indicate they have trouble making ends meet at the end of the month. The health promoter is responsible for screening patients and determining the best next steps based on their identified needs. The main responsibility of the health promoter is to identify patient needs and find solutions while supporting patients along the care pathway by navigating them to appropriate internal or external resources. POSITION RESPONSIBILITIES:·      Investigating health inequity and the relationship with social determinants of health·      Assessing social needs of patients using screening tools (in areas including nutrition, housing, transportation, finance, etc.)·      Understanding and prioritizing challenges faced by patients.·      Providing client informing care.·      Tracking patient responses and compiling data to make reportable conclusions.·      Providing follow-up sessions to assess effectiveness of navigation and program referrals.·      Travel through out the Niagara Region·      Home visits and visits at multiple clinical locations QUALIFICATIONS:·      University degree or 2-year College diploma preferably in Social Services.·      Minimum 4-6 years in a health care or social services environment.·      Minimum 1 year of case worker/intake worker experience.·      Knowledge of community programs and services.·      Analytical skills with an aptitude for critical and creative thinking to support process evaluation and problem solving.·      Ability to build strong interpersonal relationships and work with a wide range of diverse populations.·      Professionalism, strong organizational, communication and documentation skills.·      Demonstrated ability to handle a large and diversified workload with often rapidly changing priorities and challenging deadlines.·      Ability to exercise independent judgment and take action.·      Highly self-motivated and directed with keen attention to detail.·      Consistent and regular attendance required.·      Valid Ontario Drivers License
