The Community and Population Health Services portfolio supports the advancement of integrated health and social services in hospital and into the community. The Community and Population Health Services portfolio is accountable for HHS' Population Health, Bundled Care and Alternate Level of Care strategies. The portfolio provides system-wide support through the Site Administrative Managers, Discharge Specialists, ALC Navigators, Centralized Care and Transition Teams, Hospital to Home, Integrated Comprehensive Care Program and Inpatient Medical Psychiatry Consultation Liaison Teams. The McMaster Family Health Team (McMaster Family Practice, Stonechurch Family Health Centre and Canadian Blood Services) and Behavioural Supports Ontario (BSO) are aligned to the portfolio.

Position Summary

The ALC Navigator's primary focus is to expedite discharges for patients designated ALC and to hold teams accountable for continual progression of patient's discharge plans. This role also provides operational oversight of ALC designation processes across the site, ensuring that ALC patients are designated accurately, and within the timelines of Provincial Wait Times Strategy reporting requirement. The ALC Navigator addresses issues that are delaying discharge. The ALC Navigator provides a site-wide lens of ALC patients; being aware of site-based bed pressures, the navigator will collaborate with HNHB HCCSS to prioritize assessments that will result in a discharge, and works with internal and external stakeholders to resolve issues that are delaying discharges, both at a patient-specific and at a systems level.

Qualifications

  • Regulated Health Professional with Undergraduate Degree
    2. 1-2 years of computer skills (MS Word, Excel, and Power Point) EPIC, ALC-IS and IDS databases experience
    3. Minimum of 5 years experience in discharge planning, especially for patients with complex issues
    4. Minimum of 1 year presentation skills, adult education experience
    5. Daily audits of ALC-IS system, including the ability to pull reports from EPIC, ALC-IS and IDS
    6. Ability to generate weekly updates about all ALC patients in MS Excel
    7. Daily assistance to clinical teams around discharge planning of ALC patients, including connecting with community stakeholders and negotiating or advocating on patient's behalf if they have been declined to a program or service
    8. Ad hoc reports to be written for manager related to gaps in discharge planning
    9. Case synopsis writing for manager multiple times per week
    10. Excellent communication, time management and organizational skills
    11. Excellent critical thinking, problem solving, decision making and conflict resolution skills
    12. Strong leadership, interpersonal, negotiating, consensus-building and teaching skills
