What you willdo in the role

· Engage, empower, and mentor youth/young adults bypromoting ongoing maintenance of an optimal level of health, well-being, andself-esteem.

· Build an inclusive, inspirational, supportive rapportwith youth/young adults in a safe and accessible environment.

· Ensure cultural needs are met by providing access totraditional medicines, land-based activities, teachings, ceremonies, Elders,Knowledge Keepers, and/or other resources and supports.

· Provide culturally relevant and respectful service tocommunity members and/or providers in accordance with the mission, vision, andvalues of Kunuwanimano Child and Family Services

· Develop individualized plans for youth/young adultsthat support their transition to adulthood.

· Research, coordinate, plan, and/or facilitate wraparound services/programming that includes, but is not limited to, life skillworkshops, leisure activities, job preparation/readiness training, financialliteracy, and/or cultural and land-based activities, etc.

· Maintain confidential, case management for PMSSyouth/young adults assigned by ensuring accurate communication and reportsthrough timely submission of prescribed documentation (ex: statistics, logs,attendance, observations, etc.) in accordance with agency and program standard practices.

· Provide one-to one support and assistance toyouth/young adults (ex: navigation through programs, applications,transportation to appointments, shopping, activities, etc.)

· Assist with connection to current and/or formeryouth/young adults in care who meet PMSS eligibility and support voluntaryenrolment into the PMSS Program

What you bringto the table

· Post-secondary degree/diploma in social services field(alternatively, an equivalent of education and

experience may be considered)

· A minimum of two (2) years of related work experience.

· Knowledge and awareness of Indigenous culture,history, and understanding of the relevance of child welfare, residualimplications of trauma, family violence, mental health, addiction, etc.

· Understanding and appreciation of diverse Indigenouscultural beliefs, values, norms, ceremonies, traditions, and/or teachings, etc.

· Strong leadership, mentoring, and inspiring learningjourneys with ability to relate to youth/young adults.

· Produce a satisfactory vulnerability sector screeningpolice reference check.

· Valid Ontario driver’s licence, satisfactory driver’sabstract, and proof of $2M liability insurance

· Proficient in understanding and implementinginstructions, policies, and procedures.

· Exemplary problem solving, collaboration,organization, and time management skills.

· Strong oral and written communication skills inconjunction with effective case/file management skills

· Knowledge of computer technology and proficiency withvarious software packages, including but not limited to, work processing,database and/or spreadsheet applications.

· Regular travel within Agency jurisdiction with abilityto work flexible hours, including evenings and weekends, to support program andcommunity needs.
