As a Community Programs Lead with Vancouver Coastal Health you will:

  • Report to the designated Community Manager the Community Programs Lead is accountable for the provision and integration of designated community programs within the Vancouver Coastal Health (VCH) CoC including planning, developing, implementing, delivering and evaluating community program services.
  • Collaborate with VCH acute and community staff and colleagues, Physicians, community agencies and other programs/services and organizations to ensure that services are appropriately integrated on the continuum of care and implemented within allocated resources.
  • Promote and facilitate community and stakeholder participation in developing policies as well as innovative solutions to health issues as they relate to health planning, and program implementation and evaluation.
  • Oversee the day-to-day operations of designated programs, manages and supervises interdisciplnary teams, participates on and/or leads committees and task groups, and collaborates with colleagues, internal stakeholders, and researchers as required for conducting daily business and successfully achieving program goals and objectives.

Qualifications

Education & Experience

  • Bachelor’s Degree (Masters preferred) in a relevant health care profession.
  • Supplemented with five (5) to seven (7) years’ recent, related clinical or professional experience, including direct supervisory experience or an equivalent combination of education, training and experience.

Knowledge & Abilities

  • Demonstrated experience effectively developing and implementing new programs and initiatives.
  • Exhibits comprehensive knowledge of current community health program theory and practice and skillful application and development of relevant departmental policies and procedures, and the application of municipal and provincial legislative requirements related to the implementation of relevant services.
  • Applies knowledge of a wide range of cross-cultural and diversity issues and exhibits comprehensive knowledge of the social determinants of health within a health care environment. Demonstrated knowledge of population health, chronic disease prevention and primary care within community settings.
  • Demonstrated ability to engage internal/external stakeholders to develop, manage, and evaluate clinical services considering client needs, service delivery interrelationships and service potential within a complex environment.
  • Demonstrated leadership ability to foster collaborative working relationships that promote cooperative goal achievement and contribute to an atmosphere of mutual trust and respect.
  • Demonstrated analytical ability to effectively review current processes, identify areas for improvement, write reports and analyze statistical data.
  • Demonstrated ability to develop and monitor designated program budgets, monitor variances and secure additional funding from a variety of sources.
  • Utilizes sound business acumen to manage the fiscal resources in a manner that is financially responsible and consistent with the overall goals of the organization.
  • Demonstrated leadership, facilitation, collaboration and negotiation for the effective resolution of issues.
  • Applies project management methodologies in the design and development of various frameworks, tools, and techniques.
  • Strong systems-thinking, critical analysis, problem-solving and decision-making skills.
  • Demonstrated ability to apply superior judgment, political experrtise to communicate effectively, both verbally and in writing with others at all levels both internally and externally to the organization.
  • Demonstrated ability to keep informed regarding current legislation, regulations, industry trends, technology and protocols, share knowledge and support staff to integrate evidence-based practice in the delivery of quality care/practices.
  • Physical abilty to perform the duties of the position.
