Greenshield Health Digital Pharmacy is a fast growing, tech-driven, online pharmacy where innovation in patient-care comes first. With access to provincial e-health databases and new communication tools (PrescribeIT), Greenshield is redefining clinical pharmacy and becoming a key contributor in bringing hospital level of collaboration and care into the community setting. Using these tools, we are able to personalize doses to each individual and help combat medication overuse, while advancing improvement in chronic disease management and compliance. This may involve clinical suggestions to lower doses, switching to safer alternatives and even deprescribing.

We see clinical pharmacy as the future of our profession in Canada. Our clinical pharmacists are fully engaged in the delivery of expanded-scope pharmacy professional services including medication reviews, pharmaceutical opinions, minor ailment assessment and prescribing, travel health, disease state management, and prescription counselling. Our model allows us to engage our patients in bettering their health and to focus on achieving improved health outcomes. Our pharmacists are enabled to practice at the top of their knowledge, experience, and license through the dedicated clinical time, work environment, and comprehensive tools and resources. We are looking for pharmacists who will bring their drive, knowledge, and desire to take the profession to the next level to join our team.

What you'll be doing
  • Provide exceptional patient care and customer service using various methods of virtual communication (phone, virtual video meeting, email, messaging).
  • Perform and document Medication reviews and follow-ups to improve patient health outcomes by developing and acting on patient care plans.
  • Provide assessment and prescribing for all minor ailment conditions as to provincial regulation and scope of practice. Perform and document follow-up on all minor ailment assessments.
  • Produce, document, communicate, and follow-up on Pharmaceutical Opinions to resolve patient drug therapy problems and improve health outcomes.
  • Implement programs to drive critical health outcomes, improve patient knowledge about chronic diseases, and participation in their care.
  • Help to create innovative programs emphasizing the full scope of pharmacist knowledge and capabilities to help patients.
  • Perform counselling on prescriptions.
  • Thoroughly and consistently document interventions, communication, and clinical data and interpretation to ensure seamless care for all patients.
  • Communicate with the pharmacy operations team to help motivate and focus team effort on opportunities to provide patient care services.
  • Opportunity may exist to participate in social media and other patient outreach for the purposes of education.
  • Ensure compliance with all applicable Federal and Provincial laws, rules, and regulations.
  • Take on other tasks or projects to support the team, managers, and pharmacy operations & development as needed.
  • All while working in an environment designed to support you in providing high-quality pharmacy services to our patients.
Qualifications
  • Minimum Bachelor or Doctorate of Pharmacy from an accredited institution.
  • Current Ontario College of Pharmacists licensure (Part A).
  • Certified Diabetes Educator or completion of extended Pharmacist Diabetes Education from an approved program allowing for delivery of MedsCheck for Diabetes in Ontario.
  • Additional certifications or areas of specialty are an asset.
  • Excellent patient communication and customer service skills.
  • Demonstrated experience in delivering high-quality expanded-scope pharmacy professional. services; development of operations/programs/projects to improve and enhance service delivery is an asset.
  • Critical thinking and problem-solving skills.
  • Desire and drive to grow pharmacy service delivery.
  • Outstanding organizational skills.
  • Bilingualism is an asset.

·This job is on-site at our London location

·Hours: 8am to 7pm Monday to Friday (40h/week)
