Clinical Informatics Pharmacist Lead

Program Description
North Western Ontario has established a forward-thinking Digital Health Vision. This vision is:

At the centre of our Vision is a person, patient, client. Each person will be surrounded by a single, person-centric North Western Ontario Health Record (NWOHR), where clinicians work seamlessly regardless of where they work or how they are funded. Many other tools will both link into and leverage the NWOHR. As technology changes and we continue to innovate, our vision provides for those tools to also link into the NWOHR. 

The NWOHR Program will transform and enable health care across the region and stakeholders residing from the Manitoba border in the west to Manitouwadge in the east will play key roles in the in Program. Innovation in stakeholder engagement, experience working in virtual teams, and willingness to travel in the region will be required for all members of the Program team. 

The Program requires staff who are self‑motivated, energetic, independent, and who thrive in roles requiring ongoing prioritization of multiple demands. This is a Work from Home/ virtual position, therefore we are looking for a highly organized and collaborative professional, who will operate effectively in a remote working environment.

Job Summary
Reporting to the NWOHR Program Director with Clinical Direction for Work from the Pharmacy Manager, TBRHSC, as an integral member of the NWOHR Program team, the Clinical Informatics Pharmacist Lead (CIPL) is primarily responsible for the design, implementation, and ongoing support of the pharmacy information system upgrade and integrated medication record across the region for the NWOHR Program. Additionally the CIPL ensures all pharmacy information systems (drug dictionaries, clinical databases, rules, and alerts), technology, and automation are kept up to date with consistent information to ensure integration across systems for patient safety and streamlined workflow.

Responsibilities

Leadership

  • Leads the development, creation, build, testing, issue resolution, implementation, and evaluation of pharmacy information systems, including upgrades and enhancements, working with and representing regional pharmacy needs, workflow and practice. Leads the pharmacy informatics team members.

Planning

  • Plans for the upgrade of the Electronic Health Record (EHR), Medication management aspects, as part of the NWOHR Program.
  • Plans deliverables for their specific work package ensuring their achievement within the defined Program timelines and develops appropriate contingencies to mitigate liability. This includes planning for the sequencing of software development/customization to support the Program.

Medication System Management

  • Utilizes clinical pharmacy knowledge and experience in order to design, build, implement, monitor, optimize and analyze the EHR, and other relevant digital health solutions in order to ensure patient safety as a priority, and transformation of health service delivery models and practices.
  • Designs, builds, tests, and implements order sets, dictionaries and configuration settings to support patient safety, workflows, and medication management throughout the hospitals, including clinical decision support for order entry.
  • Coordinates and evaluates all requests for changes and additions to the current medication order entry system builds including patient safety, clinical evaluation, and workflow and inventory management. 
  • Ensures that the computerized provider order entry (CPOE) is implemented in accordance with recommendations and ensures that recommendations are reviewed and implemented in a timely manner according to clinical needs. Reports back on any issues, recommendations, and timelines.
  • Develops test plan and performs system testing prior to implementing dictionary and configuration changes. 

Quality Management 

  • Participates in development of best practice documentation, process improvement work flows, and other tools and resources to facilitate sustainability of the processes.
  • Assists in the development of related policies and procedures. 
  • Initiates and promotes a culture of medication delivery systems safety in alignment with Accreditation Canada and Ontario College of Pharmacists practice standards. 
  • Identifies, assesses and evaluates the impact of all medication incidents and near-misses related to the EHR and identifies and carries out system improvement plans as necessary.
  • Implements any recommendations from the Institute for Safe Medication Practice (ISMP) to prevent potential drug-related issues in the electronic medication order entry system.
  • Effectively evaluates changes to system configuration and understand their impact to pharmacy practice. 

Key Relationships and Communication

  • Works as part of an integrated team to support safe, and efficacious use of medication management component of EHR for all the regional hospital partners. 
  • Works collaboratively with internal stakeholders at each hospital, that include members of the pharmacy leadership team , the pharmacy staff, the NWOHR Program team, information services, decision support, clinical program and service directors, managers, members of the senior leadership team, educators, nurses, physicians and other direct care providers. 
  • Actively participates in Regional Pharmacy Operational and Steering Committees.
  • Collaborates with frontline clinicians to ensure a clear understanding of clinical requirements and workflow. This includes physicians, pharmacists, nurses and other allied health professionals as required.
  • Connects and collaborates with external contacts for third party system providers as required.

Education and Research 

  • Encourages and promotes pharmacy related research.
  • Assists in the development and teaching of informatics related materials for relevant front line clinicians. For example, develops training materials and trains pharmacy staff on systems supporting medication management including order-entry, decision support, clinical documentation, EHR, and other software as needed.
  • Provides pharmacy practice and clinical system expertise to trainees.
  • Communicates and disseminates all relevant information to staff through meetings, in-services, email communications or other appropriate means of communication.

Qualifications

Education/Experience

  • Minimum Bachelor of Science in Pharmacy or PharmD. 
  • Certification with the Pharmacy Examining Board of Canada. 
  • Must be a member in good standing and currently licesnsed as a Part A Registered Pharmacist with the Ontario College of Pharmacy.
  • Minimum 2 years of hospital clinical pharmacy experience.
  • Minimum 1 year clinical informatics and digital health solutions experience in a hospital pharmacy setting including project leadership, design/build and implementation of custom software in a clinical setting. 
  • Experience with project management; including development, creation, implementation and evaluation of assigned projects.

Skills/Abilities

  • Demonstrated skills in change management, problem solving, negotiation, and conflict resolution.
  • Demonstrated ability to take initiative, work independently, and as a team member in a fast paced, challenging environment.
  • Ability to manage several projects/tasks concurrently and prioritize for efficiency.
  • Outstanding attention to detail.
  • Ability to coordinate in-person and virtual meetings, conference calls.
  • Excellent ability in Microsoft Office – Word, Excel, Power Point, Outlook.
  • Strong computer experience in a medical field and information systems.
  • Background in pharmacy automation, pharmacy computer system applications, or in information technology 
  • Knowledge of hospital/clinical practice and service and understanding of general hospital operations
  • Strong clinical pharmacist knowledge and expertise.
  • Knowledge of all aspects of operational and clinical pharmacy practice.
  • Knowledge of evidence based treatments.
  • Knowledge of relevant legislation and standards.
  • Demonstrates behaviours consistent with Dryden Regional Health Centre’s Code of Conduct and Standards of Behaviour

  • Proof of full Covid-19 vaccination status to be presented prior to hire
