About the Role: As a Care Manager at our Ottawa CBI Home Health Office, you will provide clinical and service delivery leadership as a Regulated Health Care professional. Your responsibilities include nurturing relationships with stakeholders, optimizing talent efficiency, and championing a culture of engagement. You will oversee patient care planning, team building, and ensure alignment with company values and goals.
Key Responsibilities:
- Develop and revise Service Delivery Models to ensure adherence to clinical processes and care pathways
- Champion a patient- and family-centered approach in service delivery
- Monitor clinical staff performance and ensure maximum scope of practice utilization
- Identify opportunities for service improvement and implement changes as needed
- Mentor and coach care supervisors to model desired behaviors
- Promote communication and collaboration within interdisciplinary teams and with stakeholders
- Develop relationships with key stakeholders and referral sources
- Identify service gaps and develop innovative service delivery models
- Ensure staffing meets demand levels and aligns with planned growth
- Proactively identify and mitigate clinical service risks
- Provide clinical guidance, coaching, and mentoring
- Communicate best practices and ensure adherence to established guidelines
- Provide input into developing clinical education and training needs
Requirements:
- Current registration with College of Nurses of Ontario (CNO) as a Registered Nurse or Registered Practical Nurse
- 3-5 years of supervisory experience and/or experience in the home health industry
- Valid Ontario driver’s license with access to a reliable vehicle
- Strong problem-solving skills aligned with policies and best practices
