Job Description

Working together to provide outstanding integrated health care to our communities delivering best patient outcomes with exemplary standards and compassion, Muskoka Algonquin Healthcare (MAHC) is a multi-site organization that includes Huntsville District Memorial Hospital Site and South Muskoka Memorial Hospital Site.  MAJOR RESPONSIBILITIES To assess, develop, co-ordinate, implement, and evaluate stimulation and activation programs for patients encompassing 5 basic needs: spiritual, intellectual, emotional, social and physical while in hospital and to enhance and improve a patient’s quality of life through meaningful activities. This may include small/large groups, individual care and/or outings.Demonstrates knowledge of the guiding principles of client-centered care and supports this approach in practice (e.g., includes the person with stroke / caregiver in goal setting, treatment planning and decision-making).Demonstrates an understanding of the concepts that impact stroke rehabilitation.Demonstrates knowledge of the other disciplines’ roles in the management of a person with stroke and has an understanding of the concept of interprofessional collaborative practice.Recognizes the Ontario Stroke System and the Ontario Stroke Network as the model for provincially organized best practice stroke care.To provide educational in-services to staff, patients/families and volunteers in recreation and leisure and activities of daily living as needed.To participate as a member of an interdisciplinary team in the provision of patient care.To maintain charting standards according to professional and institutional standards.To maintain records of workload and service statistics and submit to administration in a timely manner.To assist in the integration of therapeutic goals from other disciplines eg. physiotherapy, occupational therapy, Speech Language Pathology into functional activities.To provide appropriate referrals and consultation to other professionals and/or agencies as required.To provide student placement experiences as caseload and resources permit. REQUIRED EDUCATION & CERTIFICATION(S)  Completion of post-secondary diploma or degree in gerontology, therapeutic recreation, or activation.CPR or willing to obtain within 1 year.Gentle Persuasive Approach (GPA) or willingness to obtain within 1 year.Valid driver's license and vehicle required. REQUIRED SKILLS & EXPERIENCE(S)Knowledge of ADL aides/physio equipment, assistive devices for mobility, communication.Knowledge of gerontology, the aging process and geriatric conditions such as dementia.Experience with stoke care would be an asset.Previous experience working in a health care setting with adults and seniors preferred.Basic computer skills including electronic health record, Microsoft office.Effective critical thinking, problem solving, organization and leadership skills. DESIRABLE EDUCATION & CERTIFICATION(S)Experience in working with the senior population, recreation and volunteer programs.Previous experience working with stroke patients an asset. ADDITIONAL PERKS AT MAHC  Staff referral program (Earn up to $1000 for each successful Permanent Part Time and Permanent Full Time new hire you refer to MAHC).Extended health benefits.Enrolment in the Healthcare of Ontario Pension Plan (HOOPP).Collaborative and supportive team environment.  WHY IS MAHC RIGHT FOR YOU? Imagine working in a place you love to visit on vacation!
